We accept new patients who will be permanently resident in our practice area (see map below) for more than three months.
Please bring the following documents with you if possible (documents marked with a * are ESSENTIAL):
- Completed new patient registration form (GMSGPR) *
- Completed Health Questionnaire form *
- Proof of Identity *
(We require at least 2 forms of identity (ID). At least one must be photographic and at least one must have your address)
- NHS card
- Drug reorder slip from you previous practice (if applicable)
Until you are fully registered with the practice, you will remain registered with your previous GP.
Until we receive confirmation of your past medical history and medication we will not be able to issue any repeat prescriptions. You should ask your previous GP for a supply.
We like to see all new patients who are on repeat medication(s). It is essential to book this appointment with a GP. This ensures that no important details are overlooked.
Patients visiting from another area in the UK for a period of less than 3 months, and who become unwell, are entitled to be seen as a temporary resident. You will be able to make an appointment in the same way as one of our normally registered patients.
If you only require a supply of your regular medication and you are normally resident in Scotland, it may be possible to obtain this from a local pharmacy under the "unscheduled care" scheme. This will avoid any delay whilst waiting for a GP appointment. If you are ordinarily resident in England Wales or Northern Ireland, it still may be possible for a pharmacist to liaise directly with your usual GP in order to arrange medication.
